Assumption of the risk

You acknowledge that you fully understand that participation in lacrosse involves inherent risks. These risks include, but are not limited to:

• Physical contact and collisions with players, equipment, or the playing surface.
• Risks of serious injury such as sprains, fractures, concussions, eye injuries, or in rare cases, permanent disability or death.
• Weather-related conditions including heat, cold, rain, or lightning.
• Travel to, from, and around the event facility.

I understand that while organizers will make reasonable efforts to provide a safe environment, these risks cannot be eliminated.

In consideration of my child’s participation in this event, I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby:

1. Waive, release, and discharge the event organizers, host facility, coaches, referees, volunteers, sponsors, and their representatives from any and all liability for injury, illness, death, or loss of property incurred during or in connection with the event.
2. Assume full responsibility for all risks of participation, whether caused by negligence or otherwise.
3. Agree to indemnify and hold harmless the released parties against any claims arising out of my child’s participation.

If I am not present, in the event of injury or illness, I authorize the event organizers, coaches, athletic trainers, or other designated personnel to provide and/or seek emergency medical treatment for my child. I understand that I am financially responsible for any and all medical treatment rendered.
